Wondering why the stern rails set up is different from mine.

That's because The owner opted for  upgraded perch seats, notice the horizontal flat white things attached top the stern stanchions ...

Yes, that's actually the ultimate reason for my question because I've always wanted to have perch seats.  Was this a factory option?   It seems like the whole rail system is different including the opening in the center.

The opening in the center is probably to allow more (or less) convenient access to the outboard, which is sort of compromised by the addition of the seats. Any marine fabricator could weld up the custom stern pulpits for you or you could do it yourself with readily available fittings (elbows, bases, etc). Looks like a good reason to go with a 23D- no worries about reaching an outboard!
